The survey is a message that automatically (Or manually) is sent to the patient asking if they had a good experience at the physician's office. By default, it will be set out 3 hours (the time is adjustable through the settings) after the appointment. Here an example:

Hi Michael. How do you rate your experience with ABC Family Practice from 0 (Terrible) to 10 (Fantastic)?

Survey's are tighly coupled with rating requests. If a patient replied 9 or 10 (Adjustable through the settings) to the survey, the rating request gets triggered.
